ACORD 67 OH, Ohio Mine Subsidence Homeowners and Dwelling
Supplement, complies with Ohio law which requires that mine subsidence insurance
coverage must be provided to dwelling insureds located in specified counties, unless the
insured rejects coverage in writing. Refer to your Homeowners or Dwelling manual for the
mandatory counties.
Ohio law also requires that mine subsidence insurance must be offered as an option to
dwelling insureds in the following counties:
- Delaware
- Erie
- Geauga
- Lake
- Licking
- Medina
- Ottawa
- Portage
- Preble
- Summit
- Wayne
Use ACORD 67 OH to apply for mine subsidence insurance in these counties.
